Personal Data Protection Notice (Appointment and Contact Form)

This privacy notice has been prepared in accordance with Article 10 of Turkey’s Personal Data Protection Law No. 6698 (KVKK) to inform you about your personal data collected through the appointment and contact forms on our website.

Data processed and purpose

Your full name, phone number, e-mail address, the subject of your enquiry and the content of your message are processed in order to handle your appointment requests, to plan the preliminary assessment process and to communicate with you. Any health information you choose to share in your message is a special category of personal data and is processed only with your explicit consent and for the purpose of the preliminary assessment.

Legal basis and transfer

Data is processed under Article 5(2) of KVKK (establishment and performance of a contract, legitimate interest) and, for special categories of data, under Article 6 (explicit consent). Apart from legal obligations, data is not transferred to third parties; it is shared with hosting and e-mail service providers only to the extent technically necessary.

Retention period

Form data is kept for a maximum of 2 years from the conclusion of the request; data that becomes part of a patient record is retained for the periods set out in the relevant health legislation.

Your rights

You can exercise your rights under Article 11 of KVKK (requesting information, correction, deletion, objection, etc.) by submitting a written application to info@yalcinkarakoca.com.
